On this first page of the chapter, we see a machine seemingly made of "metal and bones," as Tom's comment states. Some of the bones appear to be actually incorporated into the machinery, while others don't. Many of the components appear old, corroded, or damaged. I don't know enough about machines/engines to say what specifically some of these parts look like, but it's clear to me that some of these parts look like mechanical bits and some look like hydraulic bits. There are wheels, chains, and belts, but there are pipes too.
To me this looks like the answer to a question similar to "If you built a machine in the pre-electronic era using necromantic energy, what would it look like in the Ether?" Are we looking at Omega? I suspect that Omega's been around for quite a long time, hacked on by generations of Court engineers. It doesn't look like it's in great shape, though this may be the old original bits of it, and maybe more modern parts of it look better. Or maybe not; perhaps the disrepair we're seeing is a recent development, caused by either the distortion or other recent events.
The concept of an old-school machine augmented with organic parts does remind me of the New People, where Kat took some old-school robots and gave them pseudo-organic or synthetic-organic bodies and brains. Are we looking at someone's earlier attempt to do that, either with Omega or with some other project? That attempt doesn't look very successful.
Another weird idea I had was that perhaps this is a part of the Ether that came into existence when Diego started making his robots/golems, and it's in terrible shape simply because nobody's been properly dying and going back to the Ether. Maybe with Sam's essence returning, things will suddenly start looking better here. In this speculation it has nothing to do with the Court except inasmuch as Diego was involved in the Court.
